The Franciscan C.A.R.E. PLUS Program is a home-based service to help you and your family address all the challenges that prevent you from achieving healthier outcomes, aging in place and living every moment. C.A.R.E. stands for Caring through Advocacy, Resources and Education. Our program pairs you and your family with an experienced, licensed social worker who servies as your personal coach, your healthcare advocate and your expert to put you in touch with available community resources.
The Franciscan C.A.R.E. PLUS Program includes the following services:

Coordinating transitions of care from the hospital or a care facility to your home

Assistance with navigating the healthcare system, advocacy on your behalf, and coordination of your care with your healthcare providers

Disease process education to assist in long term care planning

Assistance with completing an advance health care directives to allow your family to know and follow your wishes

Caregiver education, training and support

Assistance with applications for Medicaid services

Assessments for language, cognitive or literacy barriers that may impede decision making or problem solving

Initiating and completing referrals for in-home community services (bathing, home-delivered meals, transportation etc.)

Assistance with placement in care facilities
